I don't only think of the person who gets killed or injured in these things. There is always a witness to something like this, and seeing your friend lose his head or a little girl lose her feet (foot, now - she managed to get one reattached, IIRC) would probably be very mentally scarring.

I'd hate to see anyone lose a body part like that, and even more so if I knew the person.

EDIT: Yeah, the second one is that kid's own fault. I really hope nobody on that ride saw anything, though. I didn't see anything in that article about the passengers seeing it, just that they weren't hurt.
