1. Had Terra refuse Banon's offer at the headquarters three times. This
isn't a big deal, but what this does is cause the game to skip the
Returner meeting. I usually like to watch that cutscene, but I needed to
skip it for the achievement. This means I didn't get to see Daryn's
favorite part with the crumpled piece of paper.
3. Negotiate Locke's scenario without stealing any clothes - I had never
done this before and didn't know it was possible without glitching. It
is, but it requires beating the HeavyArmor that's blocking a path at the
beginning. It has about 500 hit points. You need a decent stockpile of
Potions and Elixirs to pull this off and Locke's defense will have to be
maximized as much as possible with Armor and Relics at this point of
the game. It probably would've been easier had I leveled up Locke more
before this part, but I managed to pull it off.

Dom, did you ever experience the Phoenix Cave glitch on your cartridge? I can't recall everyone who said they did. In any case, it's probably advisable to save before going in and don't use the save while in there...just in case you encounter it again.

It seems like it trades its "hooks" for non-linear exploration, which makes the plot (which has already drastically changed tone) slow down. Thinking about it...going to various caves to look for lost party members (IIRC, you find Cyan, Locke, Gogo, Umaro, Relm or Shadow, and the new airship all by exploring caves or dungeons that are like caves), isn't as exciting or memorable as a night at an opera house, a wild ride on a mine cart in first-person perspective, a journey on a ghost train that's headed to "the other side", and a trippy underwater Mode 7 sea voyage. Antagonists that spiced things up throughout your journey like Ultros, Siegfried, and Chupon are now all relegated to the Coliseum.
It's just so weird because there's the really depressing Doma Castle
scene, then comic relief galore with Sabin - the entire Phantom Train
sequence is nonstop comedy until the end. It makes me feel guilty about
laughing when such depressing things keep happening.

All I know is that I was missing several key characters. I could not find Shadow, Relm, or Locke. Strago was in Zozo, I think, but he wouldn't talk to me. I think I was also missing Umaro, but I do recall finding Gogo. I also recall the scene where Cyan absorbs a bunch of flower bouquets, so I'm pretty sure I had him.

Dom tells me that Sabin's teacher is actually Duncan, though he has the same sprite as Banon. That seems...strange to me...
